Position Overview


Senior Care Partners P.A.C.E. is seeking a dedicated and compassionate Clinical Care Registered Nurse (RN) to provide high-quality nursing care to participants in both clinic and home-based settings. The Clinical Care RN functions independently, utilizing advanced nursing knowledge and clinical judgment to manage complex patient needs while collaborating with interdisciplinary team members to ensure exceptional care delivery.

Key Responsibilities

• Assess, triage, and respond to emergent clinical situations using sound nursing judgment.
• Monitor and manage acute and chronic medical conditions in collaboration with Primary Care Providers (PCPs).
• Implement individualized care plans to provide comprehensive participant care.
• Administer prescribed medications, treatments, and clinical procedures.
• Conduct routine home visits to provide nursing services, including:
  • Wound care
  • Lab specimen collection
  • Nursing assessments
  • Medication management
  • Other physician-ordered nursing interventions
• Maintain accurate and timely clinical documentation within the electronic medical record system.
• Collaborate effectively with licensed and non-licensed healthcare personnel, caregivers, and community partners.
• Promote participant health, safety, and quality of life through patient-centered care.

Qualifications

Required

• Associate Degree in Nursing from an accredited nursing program.
• Active and unrestricted Michigan Registered Nurse (RN) license.
• Minimum of one (1) year of experience working with frail, elderly, or long-term care populations.
• Current BLS/CPR certification or willingness to obtain certification through the organization.
• Valid Michigan driver's license, reliable transportation, and current automobile insurance.
• Must be at least 18 years of age.

Preferred

• Bachelor of Science in Nursing (BSN).
• Minimum of one (1) year of home health experience.
• Minimum of one (1) year of wound care experience.

• Compliance with organizational health and vaccination requirements, including Influenza, MMR, Varicella, Tdap/Td, COVID-19, and Hepatitis B.
