Licensed Practical Nurse (LPN)

The LPN is a medical professional cross‑trained to perform routine administrative and clinical tasks to keep the medical centers running smoothly. The duties vary with setting, and include performing clinical duties under direct physician supervision. ESSENTIAL JOB DUTIES AND RESPONSIBILITIES Measure and record patient vital signs; record patient interview and medical history. Catalog and communicate patient needs to the PCP, clinician, specialist, or other clinical personnel. Prepare patients for examinations and perform routine screening tests; assist physician with exams; explain treatment procedures and physician’s instructions to patients. Observe patients and report any changes in condition to the appropriate clinician. Perform non‑critical medical and therapeutic procedures based on medical instructions. Collect phlebotomy and other lab specimens; perform basic waived lab tests. Prepare and administer medications; change dressings, apply bandages, remove sutures; perform first‑aid procedures; use CPR skills under physician supervision. Maintain supplies, equipment, stock and sterilize instruments; practice OSHA safety standards; perform accurate, legal, and ethical documentation. Process patient phone messages, return calls and route to other team members; contact patients to obtain and relay information for the physician. Call in prescriptions to pharmacies upon physician approval. Use communication skills with appropriate medical terminology and follow professional conduct; use the ChenMed Rx system when authorized. Provide health coaching to a defined group of patients to support healthy choices; follow up via weekly calls. Perform other duties as assigned. Exceptional oral and written communication skills, time‑management and organizational skills. Collaboration with physicians, clinicians, nurses and front desk staff. Problem‑solving mindset focused on patient outcomes and team goals. Knowledge of medical products, terminology, services, standards, policies and procedures. Basic phone and computer operation. Detail‑oriented accuracy of reports and data. Organization of patient flow and systems. Calm, composed in busy or stressful situations. Proficiency in Microsoft Office Suite and related software.
