Physical Therapist- Full Time

Provides individualized physical therapy including evaluation of the patient, conception and delivery of the plan of care, follow-up evaluations and discharge planning in accordance with state law and APTA guidelines to maximize the patient's progress toward achieving functional goals. Completes accurate and timely documentation of all care provided. Enters charges for services as described by department policy. Treats patients in the acute care, LTC and outpatient setting.
