Hospice Case Manager (RN, Registered Nurse)- HomeCare

Location: 81 Meriden Ave, Bradley, CT 10003 Shift: Occasional weekends and on‑call may be required. Job Description Support patients, families, and loved ones as they go through their end‑of‑life journey. Perform a comprehensive assessment to establish goals for symptom management and end‑of‑life care with effective case management strategies. Be a member of an active interdisciplinary team that addresses patients’ needs holistically. Develop and maintain a therapeutic relationship by utilizing effective communication with patients and their families. Utilize state‑of‑the‑art technology to document skilled services provided. Qualifications Registered Nurse with a valid CT RN license. One year of RN experience; Homecare or Hospice experience preferred. Valid driver’s license and reliable transportation required.
