Position Purpose:As a member of the interdisciplinary group (IDG), provides professional nursing care to Hospice's patients. Responsible for identifying patient/family needs and for providing supportive care in accordance with the attending physician's orders, plan of care and the Hospice policies and procedures. This position is responsible for the direct care of patients and for ensuring quality and safe delivery of hospice services.Nature and Scope:The Registered Nurse Case Manager Hospice is responsible to coordinate and provide hospice services to patients who are terminally ill and their families, complying with the agency policies and procedures. The incumbent must be highly motivated and demonstrate critical thinking skills. The position requires independent judgment and assessment skills. The ability to drive and be on-call are components of this position. The incumbent must demonstrate the knowledge and skills necessary to provide care based on physical, motor/sensor, psychosocial, and safety appropriate to the age of the patients served.The incumbent provides care under the direction of the attending physician, the Hospice Manager and in compliance with the Nevada Registered Nurse Practice Act. This position requires a scientific knowledge base and ongoing growth and learning. When possible the family or those significant to the family are considered as part of the assessment, planning, intervention and evaluation efforts of the Registered Nurse (RN). The RN coordinates care between other professionals as needed. Intensive continuous mental effort is required to provide optimal safe patient care and the same mental effort is required when interacting with internal and external contacts, exercising judgments and making decisions. Graduate from an accredited School of Nursing; Bachelor's Degree preferred. Appropriate education to obtain and maintain State of Nevada Registered Nurse licensure.Experience:At least one year of acute care experience and/or one year of home health or hospice preferred. Understanding of Hospice philosophy required. Demonstrated ability to assess and respond to the needs of patients and families in varied settings and to cope with emotional stress. Must be able to function in a practice environment with minimal direct supervision, accepting personal responsibility for maintaining a professional relationship with patients and their families.License(s):Ability to obtain and maintain a State of Nevada Registered Nurse license.Valid State of Nevada or California driver's license and ability to pass Renown Health's Department of Motor Vehicle Report criteria. Must be able to transport self to patient's home/facility. May be working in multiple locations in a workday.Certification(s):Current BLS certification by The American Heart Association standards.Computer / Typing:Must be proficient with Microsoft Office Suite, including Outlook, PowerPoint, Excel and Word and have the ability to use the computer to complete online learning requirements for job-specific competencies, access online forms and policies, complete online benefits enrollment, etc. Unit Description Provide overview of position/department, common procedures, key technology, etc This position will act as a hospice visit RN. Primary role will include completing in-home nursing care for terminally ill patients, medication management, providing comfort-focused care, and providing patient and family emotional support. RN may be assigned a small caseload of patients to oversee and/or complete standard visits for other case managers. Documentation is completed in Epic- laptop, phone, and clinical supplies will all be provided. After hours call is required (3 nights per month). Most calls are related to medication/symptom questions or patient passing. Home visits after hours may be needed during on-call shift if issue cannot be solved over the phone. RN will need reliable car to travel from home to home. Non-Negotiables Describe necessary experience level, licensure, certifications, education RN Schedule Describe shift/schedule structure Business hours 8A-4:30P. Open 7 days/week, so various schedules are available. Will be required to take minimum of 3 nights per month of after-hours call. Support over the weekend would be welcomed, but not required. Service Agreement Key timelines to be followed by Leadership/Recruitment Leadership - • Review submissions within 24 to 48 hours of receiving • Respond to Recruitment with all offers *Goal of making at least 50% of offers within 2 days of receiving submissions; currently at 28.76%* • Speak with each traveler at the 6 week mark about converting to core staff Recruitment - • Post approved reqs within 24 hours of approval • Make job offers by close of business each day • Recruitment to "re-announce" jobs once per week to reflect refreshed post to agencies • Send extension requests to Director/CNO for approval two times per week • Escalate time off requests to Managers/Directors for approval
