As a Licensed Practical Nurse (LPN), you will provide direct nursing care, administer medications and treatments, monitor residents' health, collaborate with the interdisciplinary care team, and help ensure each resident receives the highest quality of care in a safe, supportive environment.

We are currently seeking a Licensed Practical Nurse (LPN) to join our growing team.

Job Responsibilities and Duties:

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings

• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications

• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation

• Provide wound care: follow protocols, irrigate, pack, and remove dressings

• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies

• Respond effectively to emergency situations
