Nursing Supervisor

Job Summary BASIC FUNCTION AND RESPONSIBILITIES: This position provides oversight of all patient care services and personnel. Responsibilities include: oversight of registered nurses, licensed practical nurses, physical therapists, occupational therapists, speech-language pathologists, social workers, home health aides, customer service representatives, quality department staff and others as determined by the Clinical Nursing Director/Administrator; facilitating the delivery of effective, cost-efficient, high quality, professional home care services; implementing department policies and procedures in accordance with regulatory, accreditation and practice standards; and clinical management of all relevant adult medical-surgical clinical programs in collaboration with the Clinical Nursing Director/Administrator and others. Serves in the role of the Nursing Supervisor. CHARACTERISTIC DUTIES AND RESPONSIBILITIES Develops, evaluates, and modifies a unit staffing plan to meet patient care needs. Provides direct supervision and evaluation of staff including: registered nurses, licensed practical nurses, physical therapists, occupational therapists, speech-language pathologists, social workers, home health aides, patient service associates, and on-call triage nurses. Understands and implements bargaining agreements for UMPNC and AFSCME employees, and manages other staff through the Standard Practice Guide, as well as HCS and MVN policy. Ensures that consistent practices are followed. Ensures Home Health Aide competency and compliance within service/education regulations. Supervises staff and evaluates job performance on a regular basis through objective review. Completes employee performance appraisals annually. Understands and interprets/implements department and Michigan Medicine policies and procedures guidelines in a consistent manner. Provides oversight of all patient care services and personnel, including making patient and personnel assignments, coordinating patient care and coordinating referrals. Evaluates, directs and monitors timeliness of documentation of services by field staff. Assures that patient needs are continually assessed and oversees the coordination of patient care. Knowledgeable about patient caseloads and practice issues of staff. Assures that plans of care are individualized, clinically appropriate, updated as needed and implemented in a cost-effective manner. Communicates with all physicians involved in the plan of care to ensure coordination of all patient care services and interventions. Manages team performance based on established metrics, using data in decision-making. Holds staff accountable to work and productivity expectations. Acts as or appoints a case manager for all patients and ensures that patient care is being provided in consideration of quality and financial metrics. Collects, reviews, and evaluates data for issues regarding management of individual teams and guiding decision-making. Performs skilled intermittent visits when necessary to meet the needs of patients. Works collaboratively with other Home Care Services staff, care management, intake, quality, billing and related departments to coordinate care, build relationships and facilitate effective discharge planning and provision of patient care. Provides weekend, holiday, and evening supervisory coverage as assigned. Serves in the role of the administrator when the administrator is not available. Consistently demonstrates appropriate and professional verbal and written communications. Reviews staff and patient incidents; develops action plan with staff and implements appropriate measures to correct incidents and prevent them from recurring. Provides exemplary customer service and service recovery to patients, patient representatives (If any), families, and internal staff. Assists in developing, implementing, and participating in staff education programs, policies, guidelines, orientation and in-services. Guides and counsels staff in a timely fashion to promote an understanding of, and compliance with, department policies, guidelines and practice standards. Conducts disciplinary activities with sensitivity for all staff and in accordance with relevant labor relations standards. Demonstrates desired results in achievement of all relevant strategic, budgetary and operational targets and objectives. Receives, investigates and resolves patient concerns and complaints. Participates in development and ongoing review of the Quality Assurance and Performance Improvement program {QAPI). Ensures priority initiatives are addressed timely. Participates in accreditation survey readiness activities and the survey process. Assists in the planning, organization, implementation, evaluation and supervision of student training and provision of care and services. Works with the human resource department regarding need for incremental staff, interviewing and screening of applicants, and assisting in orientation of new staff to department standards. Other Related Duties: Practices within the scope of license, certification, or registration. Complies with all applicable continuing education requirements. Complies with all University of Michigan and departmental standards and expectations. Supports the department's commitment to continuous improvement activities and principles through positive and respectful contribution in all problem-solving and change processes at Home Care Services. Performs other duties as assigned. Required Qualifications* Current licensure as a Registered Nurse in the State of Michigan. Bachelor's degree in Nursing OR Master's degree in Nursing Minimum 5 years RN experience Minimum 2 years of recent (within the last 5 years) RN experience in home health care Ability to travel from point A to point B throughout the workday as assigned Completion of a Nurse Leadership certification within two (2) years of accepting this position or when eligible to certify, whichever comes first. Certification is expected to be maintained while in this leadership role. Examples include Certified Nurse Manager and Leader (CNML) or, Nurse Executive (NE-BC). Physical Requirements The duties include performing skilled physical assessments and various skilled nursing activities, such as changing dressings, providing medication via IVs and injections, drawing blood, assisting with personal care, exercises/ambulation, auscultation of lung and bowel sounds, etc. The duties also include the use of equipment for client care, such as oxygen and suctioning equipment, medication dispensers, laboratory screening meters, and IV pumps. This position requires knowledge of the use of laptop computers, and cell phones for documentation and communication. These duties require fine motor coordination/dexterity, the ability to grasp, vision with or without corrective aids adequate for fine motor tasks, client assessment, and driving. The job requires the physical demands of occasional lifting greater than 35 pounds, reaching, pushing, walking, going up and down stairs, driving, etc., with or without the use of assistive/adaptive devices. Employee drives personal or University owned vehicle to various work site locations on a regular basis including in-town and highway driving in good and inclement weather. Work Environment The job requires the health care professional to work with the patient in the patient's home. Must be able to travel from point A to point B throughout the workday in all kinds of weather as assigned. Environmental factors of the patient's home are not within the control of Michigan Medicine (i.e., whether or not there is a smoker in the home, pets, apartment complexes, home entrance, etc.). Occupational Hazard Information Staff members may have routine exposure to all chemical inventory, inclusive of hazardous materials, housed at all work sites for Home Care Services, including but not limited to: the office, hospital, clinics, vehicles and warehouse. Chemical inventory reference materials are available to all staff in the form of hard copy and on-line Material Safety Data Sheets. Staff may carry out assignments in patient's homes, hospitals, clinics and HCS offices. Staff may drive personal or University owned vehicle and walk on uneven surfaces to these various work site locations in all weather conditions on a regular basis.
