Family Mentor

$25.05 - $33.24 per hour

Central City Concern

This position supports recovering and formerly homeless families living in Alcohol and Drug-Free Community (AFDC) housing by providing advocacy, family mentoring, and other support. The Mentor acts to ensure the stability of families and assigned properties, and will be familiar with and able to access resources for residents, including childcare, employment assistance and training, transportation and other services. The Family Mentor will be someone who is comfortable and advanced in his or her own recovery, and will have a good understanding of professional boundaries and appropriate behavior. Non-binary individuals will have the skills needed to support and assist residents in maintaining recovery, retaining housing, parenting safely and effectively, and achieving self-sufficiency. Schedule: Monday - Friday, 9:00am - 5:30pm Location: Cambridge Court (5224 N Vancouver Ave. Portland, OR 97217) Compensation: $25.05 - $33.24/hr *Final comp will be determined based on relevant experience, education, credentials/licensure, and internal equity. MINIMUM QUALIFICATIONS:

  • GED or High school Diploma required
  • Peer Support Specialist or Certified Recovery Mentor required.
  • Must be registered with MHACBO as a Certified Recovery Mentor OR
  • Must be a certified Peer Support Specialist (or Peer Wellness
Specialist) and be registered as a Traditional Health Worker on the THW registry. * Experience in human or social services, working with low-income families preferred. * Experience working or volunteering with children (e.g., school, daycare, youth programs, etc.) preferred.
  • Flexible work schedule required; may include some evenings and weekends.
  • Must pass a pre-employment drug screen and background check. This includes
clearance by the DHS Background Check Unit * Must have standard First Aid Certificate and current CPR/Adult and CPR/Infant-Child certification prior to start. * Must possess a current driver’s license, access to a vehicle, qualify as an Acceptable Driver as designated in Central City Concern’s Fleet Safety policy.  Must pass an initial driver training within 60 days of being an approved driver and continued recertification training.  Must maintain vehicle insurance coverage of a minimum of $100,000/$300,000 personal auto liability coverage. * Will be required to carry an agency cell phone for work use. Cell phone will be provided by Central City Concern. * Ability to adhere to Central City Concern’s drug-free workplace which encourages a safe, healthy and productive work environment and strictly complies with the Drug-Free Workplace Act of 1988. An employee shall not, in the workplace, unlawfully manufacture, distribute, dispense, possess or use a controlled substance or alcohol * Must generally have the ability to bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects and climb stairs, with reasonable or no accommodation.

DESCRIPTION OF DUTIES:

* Maintain active caseload and function as mentor and resource specialist to families in ADFC housing complexes.
  • Connect families to recovery support systems and programs.
  • Conduct initial assessment of family competencies and needs, according to FAN
program guidelines * Work collaboratively with families to develop FAN Service Plans (FSP), monitor FSP’s, and formally review FSP’s every 90 days.
  • Identify and facilitate clients’ access to other community resources.
  • Develop and maintain current resource directory of available services,
including childcare, housing, clothing, furniture, food programs, recreation, transportation, legal services, employment and training. * Coordinate property maintenance, repairs, management, and tenancy with CCC Family Housing Portfolio Manager as needed, and promptly relay any concerns regarding property. * Work with referral agencies (including residential and outpatient treatment providers) to facilitate family transition plan.
  • Ensure stability of clients and housing complexes in caseload.
  • Cultivate and maintain positive working relationships with families, and
other community resources. * Monitor the safety of all residents, including children, and inform authorities about unsafe situations when necessary
  • Closely follow FAN protocols for emergency situations and critical incidents
  • Develop and maintain good working relationships with DCJ, DHS, child welfare
and other agency staff to maximize client success.
  • Maintain records and documentation in accessible and legible form.
  • Ensure that all records are current, and client interactions meet applicable
State and Federal guidelines and regulations.
  • Carry a case load of High Resource using families.
  • Attend planning and wrap around meetings with community partners.
  • FAN Mentor will identify and meet any remaining needs, such as accessing food
programs, finding clean clothing, childcare, treatment and recovery activities, maximizing services from DHS, Child Welfare and other agencies, transportation, and accessing other services (such as mental health treatment and legal services) as needed. * Family Mentor will help residents develop better coping and parenting skills, keep them engaged in recovery and learn (or re-learn) skills needed to survive in mainstream society, like how to shop and prepare nutritious meals on a tight budget, managing family finances, and developing job skills.
  • Perform unit lock outs with Property Management.
  • Report vacancies to Program Coordinator within 24 hours of move out
  • Report all physical property deficiencies to be placed as work orders to
Housing * Assist Occupancy with residents who do not comply with re-certifications in a timely manner.
  • Carry a cell phone and be on-call after hours for emergencies
  • Contact Property Management when after hour emergencies occur.
  • Adhere to all state and federal privacy regulations, including HIPAA and 42
CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security.  Support compliance with all privacy and security requirements pursuant to community partners’ and outside providers’ patient confidentiality agreements, including privacy and security requirements for EMR access.  This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee. * Other duties as assigned.

SKILLS AND ABILITIES

* Understanding of chemical dependency as a disease, including treatment and intervention methods. * Ability to understand and support self-sufficiency, re-unification, treatment and discharge plans developed by other agencies * Knowledge, training and/or experience in infant, child and adolescent development, the ability to model developmentally appropriate interactions with children and adolescents, and the ability to spot developmental concerns that may call for further assessment * Knowledge, training and/or experience regarding positive parenting and healthy family dynamics * Ability to work both as a team member and independently to achieve program goals. * Understanding of HIPPA, confidentiality and boundaries, and a demonstrated ability to incorporate into daily job performance. * Ability to understand and implement self-sufficiency, treatment or discharge plans developed by other agencies. * Knowledge, training and/or experience in early childhood developmental issues and healthy family dynamics, and ability to model or communicate these to clients. * Ability to clearly communicate expectations and consequences to clients/tenants and their families and guests.
  • Ability to manage complex and multiple tasks and prioritize appropriately.
  • Demonstrated knowledge of community and social agencies, and self-help
groups.
  • Sufficient manual dexterity and physical ability to perform assigned tasks.
  • Ability to communicate clearly and concisely both verbally and in writing.
  • Comfortable working with people from a diverse range of ethnic and
socio-economic backgrounds, including those with dual diagnoses or other disabilities who may display hostility.
  • Ability to respond appropriately to residents’ complaints and grievances
  • Ability to recognize and acknowledge the limits of own ability and expertise,
and a willingness to seek information, help or support whenever appropriate * Detailed oriented; ability to maintain accurate records and necessary paperwork. * Knowledge of and ability to utilize de-escalation techniques, when encountering volatile situations. * Ability to effectively communicate with clients, their guests and children; co-workers; corrections personnel, police, child welfare workers and other community referents. * High degree of comfort and sufficient advancement in personal recovery to appropriately facilitate clients in theirs. * Demonstrated understanding of professional and personal boundaries in the workplace.
