Job Description – Clinical Pharmacist – Emergency Medicine – PRN (As Needed) Overview The emergency medicine clinical pharmacist provides all pharmaceutical services in the Emergency Department and across the organization’s emergency and trauma services. This high‑risk clinical environment includes point‑of‑care medication ordering, dispensing, and administration, frequent verbal orders, a wide variety of time‑sensitive medications, and high‑risk intravenous therapies such as inotropes, sedatives, and other critical drugs. As a member of the multidisciplinary team, the pharmacist promotes safe, rational, and efficient medication use, reduces costs, oversees medication reconciliation performed by technicians, and may cross‑train in the main pharmacy when needed. Schedule Variable schedule. Twelve‑hour shifts with at least one shift per month: either 3:00 AM–3:30 PM or 3:00 PM–3:30 AM. Required Qualifications Education : Completion of a four‑year accredited pharmacy program (Required). Minimum Work Experience : 1 year successful completion of PGY‑1 (pharmacy practice residency with pediatric experience) or equivalent (Required); 1 year PGY‑2 in critical care or emergency medicine (General Pharmacotherapy PGY‑2 acceptable) (Preferred); 3 years Pediatric Emergency Room Pharmacy experience (Preferred). Licenses & Certifications : Current Washington DC pharmacist license (Required); Basic Life Support for Healthcare Provider (Required); Pediatric Advanced Life Support (Past 180 days) (Required); Cardio‑Pulmonary Resuscitation (Past 180 days) (Required); Advanced Cardiovascular Life Support (Past 180 days) (Required); Board of Pharmacy Specialties Certification (1 year) (Required). Skills & Knowledge : Proficient in medication management and pharmacology for airway management/rapid‑sequence intubation, ambulatory care, cardiology, critical care, infectious diseases, pediatrics, procedural sedation, psychiatry, toxicology/drug abuse/overdose, disaster management, and related areas. Functional Accountabilities Medication Processing Interpret medication orders, profile orders for appropriateness and oversee packaging, labeling, and distribution of medications. Provide pharmacy review of high‑risk medication orders prior to administration. Assist in preparation of high‑risk, time‑sensitive therapies in the emergency department. Assess the effectiveness, safety, and affordability of patient medications. Drug Intervention and Education Perform drug therapy monitoring and intervene on dosing, administration, interactions, side‑effects, and monitoring issues; complete therapeutic drug monitoring consults and adverse drug reaction reports as assigned. Provide accurate and timely drug information on medication use, dosage, side‑effects, and composition to physicians, nurses, and patients/families; including patient‑specific medication use teaching for discharge medications when appropriate. Participate in transitions of care and assist with patient access to medications. Interpret research medication orders, ensure accurate preparation of investigational drugs, and maintain complete records. Participate in quality improvement activities – floor inspections, medication use evaluations, medication management programs, clinical guidelines, critical pathways, disease‑management, drug‑use programs, and documentation of interventions. Support contributions to pharmacy and medical literature; participate in research when applicable; maintain knowledge in area of expertise. Participate in medical and trauma resuscitations within the ED and assist in code situations outside the ED; the ED pharmacy team ensures continuous 24/7 code response. Assist with responding to drug‑information questions for critical and acute care patients during periods without decentralized pharmacy services (e.g., holidays, evenings, weekends). Participate in development, implementation, and review of Cerner order entry and orderset build. Pursue independent projects, teams, and committees; attend hospital committee meetings as needed. Engage in continuing education, in‑services, and workshops to maintain current knowledge and enhance professional growth. Focus on cost avoidance and savings due to medication use in the ED. Complete medication‑history documentation and actively participate in the clinical pharmacy program as an expert consultant. Assist in training pharmacy staff on code response and emergency preparedness. Prepare and collect data for MUEs, drug monographs for the Pharmacy & Therapeutics Committee, and other special projects assigned by the ED Pharmacy Supervisor. Perform other duties and special projects as required. Supervision and Training Oversee Pharmacy Technicians; direct and assist to ensure rapid medication‑order turnaround time. Provide didactic and experiential training for Pharmacy Residents and Students. Support professional development and competency of clinical staff; serve as an educational resource. Teamwork and Communication Demonstrate collaborative and respectful behavior. Partner with all team members to achieve goals. Receptive to others’ ideas and opinions. Performance Improvement and Problem Solving Contribute to a positive work environment. Demonstrate flexibility and willingness to change. Stop when uncertain and take time to resolve the situation. Communicate accurately, clearly, and promptly in both verbal and written forms. Actively promote safety for patients, families, visitors, and co‑workers. Focus on details and practice stop‑think‑act‑review for self‑checking. Facilitate proper information transfer regarding medication use for patients transferred to inpatient registry from the ED.
