LVN - HOME HEALTH

Position OverviewPosition Summary

As a Licensed Vocational Nurse (LVN) – Home Health , you will provide skilled nursing care and related services to patients in their homes under the direction of a Registered Nurse (RN) or physician. You will play a vital role in implementing individualized care plans, educating patients and families, administering treatments, and supporting positive patient outcomes.

This position may include participation in an on-call rotation based on agency needs and location requirements.

Responsibilities

Deliver skilled nursing care under the supervision of an RN
Conduct routine patient home visits according to agency policies
Assist in developing and implementing individualized care plans
Monitor patient conditions and communicate changes to the care team
Administer medications and prescribed treatments
Perform clinical procedures including:
- Injections
- Phlebotomy
- Drug screenings
- Wound care and sterile dressing changes
Educate patients and families regarding treatment plans and self-care
Support discharge planning and patient goal achievement
Coordinate care with interdisciplinary team members
Maintain accurate clinical documentation
Order and restock medical supplies as needed

QualificationsRequired

Current Licensed Vocational Nurse (LVN) license in the state of practice
Current BLS (Basic Life Support) certification from an approved provider
Strong clinical assessment and patient care skills

Preferred

Bachelor's Degree
Arrhythmia certification
Valid driver's license
Previous home health experience
