This is a home-visit hospice model focused on delivering compassionate, high-quality end-of-life care.
Practice Structure:
• 2 3 home visits per day
• Strong interdisciplinary hospice team
• Support from attending physicians and consultants
Responsibilities:
• Hospice assessments and care planning
• Advanced directive discussions
• Symptom management
• Coordination with nursing and family caregivers
• Promotion of comfort-focused care
Compensation & Benefits:
• $115K $130K salary
• Laptop and cell phone provided
• Mileage reimbursement
• $5,000 annual CEU allowance
• Full benefits package
• Retirement plan
