Growing together.The Annual Wellness Visit Coordinator (AWVC) is responsible for reviewing a patient's chart and paperwork in preparation of the patient's wellness visit to ensure they are up to date with all medical information, procedures, vaccinations, testing, etc. and providing this information to the provider team.Primary Responsibilities:Accurately documenting medical history, and patient forms information for the providerIntake of Vitals (B/P, Height, Weight, Glucose, HR, BMI)Education to patients on procedure, treatment, medications and assisting with questions from the Annual Wellness PacketMaintain patient confidentiality on all encounters (HIPPA)Partner with other departments to facilitate quality of care (radiology, lab, pharmacy, nuclear medicine etc.)Other duties as assigned such as new models of quality care and value if they are due for any testing they inform the provider etc.Administrative Tasks:Schedule patients for follow up/lab appointmentsScan in patient information/ documentsMaintain clinical compliant logs for Practice Health & SafetyAssisting providers by closing care gaps on Optum ProPortal, Cozeva and other insurance requirements i.e. (BCBS, Humana, Aetna, etc. as plans and value grow.)Clinic Operations/General Responsibilities:Float to other clinics as neededCross training on Nurses Station, Lab, Front Desk and ScribeProactively identity any safety concerns to Site ManagerTrain other clinical staff on job dutiesYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:High school degree or equivalentCertified Clinical Medical Assistant (CCMA)/ Certified Residential Medical Aide (CRMA)- Required or a Licensed Practical Nurse (LPN) or Registered Nurse (RN) License or education of higher1+ years working in a healthcare settingProven knowledge of federal, state and local laws and regulations including OSHA and HIPAA requirementsProven knowledge of computerized information systems (EMR) used in clinical management settingsProven knowledge of medical terminologyPay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The hourly pay for this role will range from $16.00 to $29.00 per hour based on full-time employment.
