Luminos Hospice Physical Therapist - PRN

Description A registered professional who provides coordinated care to patients of all age groups. Plans, implements, and evaluates patient care plans to maintain patient well-being. Plans, organizes, and conducts physical therapy treatment program based on the medical referral and their evaluation. Collaborates with Interdisciplinary Group/Team (IDG/IDT) Requirements Education : Graduate of a Physical Therapist Education Program that is approved by the Commission of Accreditation in Physical Therapy Education (CAPTE) or a successor organization of CAPTE. Doctor of Physical Therapy required. *Note: Exceptions may exist for education and licensure prior to 2009 and if educated outside of the United States. (42CFR 418.114(b)(7)(ii-vii). Licensure : Current state license as a physical therapist, or a multi-state license issued by a state member of the Physical Therapist Licensure Compact (PTLC) if practicing in a state member of the PTLC to practice physical Therapy. Experience : Two years’ experience as a physical therapist in an acute care or rehabilitation setting recommended. Skills : Therapy skills as defined as generally accepted standards of practice.
