• Weekly Estimated Pay: $2,120 - $2,230
• Schedule: Monday-Friday, 8:00 AM - 4:30 PM
• Shift Duration: 8 hours
• Hours per Week: 40
• On-Call Requirement: On-call rotation during the week and weekends
• Travel Type: Travel assignment
• Mileage: Reimbursed between visits, from first patient to last patient

Job Requirements

• Active Registered Nurse (RN) license
• Minimum of 1 year of home health or hospice nursing experience
• Experience with OASIS visits
• Familiarity with Homecare Homebase (HCHB)
• Associate degree required
• Strong clinical assessment, documentation, and patient education skills
• Comfort working independently in a home-based care environment
• Wound care and IV experience preferred

Responsibilities

• Complete home health RN visits , including start-of-care and follow-up visits
• Perform OASIS assessments accurately and efficiently
• Provide wound care and support for IV patients
• Document patient care thoroughly using HCHB
• Collaborate with the care team to coordinate high-quality patient outcomes
• Participate in on-call coverage as scheduled
• Deliver compassionate, patient-centered care in the home setting
