- Supportive Staffing Ratios
- Provide direct nursing care to residents according to care plans and physicians' orders.
- Administer medications and treatments accurately and in a timely manner.
- Monitor and document residents' health status, reporting any significant changes to the RN or physician.
- Assist with resident assessments and the development of individualized care plans.
- Supervise and direct CNAs and other nursing assistants in delivering daily care.
- Ensure all documentation is complete, accurate, and in compliance with facility policies and state/federal regulations.
- Respond to emergencies and follow proper protocols.
- Support residents and families with health education and emotional support.
- Maintain compliance with infection control, safety, and HIPAA standards.
- Participate in interdisciplinary team meetings, audits, and in-service training.
- Valid LPN license in the state of employment
- Graduate of an accredited Licensed Practical Nursing program
- CPR/BLS certification (required or able to obtain upon hire)
- At least 3 years of nursing experience (long-term care preferred, but not required)
- Ability to communicate effectively and compassionately with residents, families, and team members
- Familiarity with electronic medical records (EMR) systems preferred
- Strong critical thinking, organizational, and time-management skills
