Career Growth: Opportunities with the largest employer in South Dakota. Job Summary Position Title: Licensed Practical Nurse (LPN) – or Registered Nurse (RN). Provides each resident with professional nursing care, assessment and evaluations within scope of practice. Responsibilities Deliver professional nursing care, including assessment, diagnosis, planning, implementation, and evaluation. Develop nursing care plans by obtaining medical histories, examining residents, documenting physical condition, psychological status, and social factors. Provide physical, emotional, psychological, and spiritual support to residents, families, and friends. Review physician prescriptions, communicate nursing care plans and pertinent information to team members, and coordinate with other healthcare providers. Teach residents and families about conditions, medications, and self-care skills. Evaluate care by measuring health outcomes against standards, reviewing records, and re‑evaluating care objectives. Maintain safe, secure, and healthy resident care environment by adhering to infection‑control policies, medication administration and storage procedures, controlled substance regulations, and organizational standards. Maintain equipment and supplies, ensuring readiness and proper operation. Coordinate admissions, discharges, and transfers to and from the facility. Direct, plan, and coordinate workflow of nursing assistants and other nursing personnel. Maintain competence in nursing capabilities outlined by leadership and comply with safety principles and regulations (CMS, Joint Commission, DHHS, OSHA). Qualifications Required: Active LPN license in the state of practice (upon hire). “Registered Nurse” license also accepted. Preferred: 1–3 years experience in a clinical or long‑term care environment. Commitment to applying Avera’s mission, vision, core values, and social principles daily.
