Description Transitional Care Technician We are seeking a compassionate and detail-oriented Transitional Care Technician to join our healthcare team. This vital role involves supporting patients as they transition from hospital to home or other care settings, ensuring a smooth and safe process. Key Responsibilities Assist patients with daily activities and mobility during transitional care periods Provide emotional support and education to patients and their families about post-discharge care plans Collaborate with healthcare professionals to coordinate patient care and ensure adherence to treatment protocols Monitor patient conditions and report any changes or concerns to the nursing staff Help with basic medical tasks such as vital signs measurement and medication reminders Maintain accurate documentation of patient interactions and care activities Ensure a clean, safe, and comfortable environment for patients during their transition Skills and Qualifications High school diploma or equivalent; additional healthcare certification is a plus Prior experience in patient care, healthcare support, or related fields preferred Strong communication and interpersonal skills Compassionate, patient-focused, and able to handle sensitive situations Ability to work effectively as part of a multidisciplinary team Basic knowledge of medical terminology and healthcare procedures Flexibility to work various shifts, including evenings and weekends Join our dedicated team committed to providing exceptional transitional care and improving patient outcomes.
