Pediatric Occupational Therapist $500 Bonus- EI/Preschool

Occupational Therapist *$500 sign-on bonus after 90 days of employment with active cases (10hour/week minimum)* Details about this opportunity: Location: Farmingdale and/or Islandia Status: Fee for Service or Full-Time split with both sites Hours: During and/or afterschool hours, flexible scheduling Compensation:

• Early Intervention: $64-$75/hour
• Preschool: $70/hour

OR

• Full-Time, Salary $62,000-$65,000
• *Please note this is a range. Rates are determined by the service provided,

professional experience and license/certifications held. In addition the position posted may be either full-time or fee-for-service (FFS), depending on the current caseload which will be discussed during the interview. • Administer and write up occupational therapy evaluations
• Ongoing communication with lead supervisors & coordinators, specifically

related to case management * Maintain professionalism with families, school teams, and other professionals

• Maintain accurate and timely attendance and billing logs
• Create and maintain schedules with assigned individual families/school

personnel

• Collects data on each IFSP/IEP goal and reports on progress quarterly
• Provide input and attend/teleconference initial CPSE/CSE meetings and/or

annual reviews About You: If you have these attributes:

And this background:

• Graduate from an accredited university with a degree in Occupational Therapy
• Current New York State Occupational Therapy License
• Bilingual is a plus
