Somatus - Revolutionizing Kidney Care
Somatus seeks a Patient Health Advocate to support CKD/ESKD patients, partnering with physicians and care teams to coordinate care across settings. You will conduct outreach, scheduling, education, and transition-of-care activities, including home visits and in-office coordination, to reduce readmissions and improve patient outcomes.
