Position Overview

As a Home Health Physical Therapist, you will:

• Provide individualized, one-on-one therapy services in patients’ homes

• Evaluate patients and develop customized treatment plans

• Monitor patient progress and adjust interventions as needed

• Educate patients and caregivers to maximize outcomes

• Collaborate with physicians and interdisciplinary healthcare teams

Qualifications

• Doctorate or Degree in Physical Therapy (DPT or equivalent)

• Minimum 2 years of clinical Physical Therapy experience
  
  
• Home Health experience preferred

• Active California Physical Therapist License

• Valid California Driver’s License

• Current CPR certification

• Current TB test and Professional Liability Insurance

• Strong communication, documentation, and organizational skills

• Ability to work independently and manage a flexible caseload

Schedule

• Choose your own hours

• Flexible weekday and weekend opportunities available

• Work location: On the road / patient homes

Compensation & Benefits
