Location: Winston-Salem, NC 27103

Pay: $58.65/hour

Schedule: Mon-Fri, 8:00am-5:00pm | No on-call

Counties Covered: Davidson & Forsyth (field-based home health)

Mileage: $0.43/mile reimbursement

Position Overview

We're hiring a Registered Nurse to support high-risk members through comprehensive assessments, individualized care planning, and care coordination across the continuum. This role focuses on strong clinical judgment, documentation, and collaboration with providers and interdisciplinary partners to improve outcomes and prevent avoidable events.

Key Responsibilities

Perform comprehensive assessments for high-risk members/patients
Collaborate with PCPs/providers to implement individualized care plans
Coordinate transitions of care with physicians, social workers, discharge planners, and other partners
Identify and connect non-medical supports (transportation, housing resources, etc.) to improve compliance
Engage specialty resources as needed to drive best outcomes
Maintain detailed clinical/functional/outcome documentation
Identify health promotion and illness prevention opportunities
Advocate for patient privacy and confidentiality

Job Requirements

Required Qualifications

Active, unrestricted RN license (North Carolina)
Nursing degree (Associate or Bachelor's)
Home Health experience with HCHB (Homecare Homebase) & OASIS experience (certification preferred)
Case Management experience (Case Management certification preferred)
