Key Responsibilities: As a Home Health RN, you will: Conduct Home Health Nursing Assessments to determine equipment needs, participant preferences, and care goals. Deliver and document home health nursing interventions in alignment with care plans, including medication administration, physician orders, and maintaining a safe environment. Educate participants, families, and caregivers on disease processes, self-care techniques, and prevention strategies . Track and monitor Home Health scheduling in collaboration with the Home Health Manager or scheduling point of contact. Work closely with Physicians and the Interdisciplinary Team (IDT) to ensure smooth care transitions between hospitals, skilled nursing facilities, and home settings. Basic Qualifications: Graduate of an Accredited School of Nursing . Unencumbered RN License required. Minimum one year of nursing experience in a clinical setting, preferably with a frail or elderly population. Home health agency experience is a plus.
