As a therapist at CenterWell Home Health, you'll help patients regain strength, mobility and independence-all from the comfort of their homes. You will report to the Clinical Manager.

This opportunity will require an alternative Baylor/Weekend full-time schedule, to be discussed during interview (i.e. 12 hour Fri/Sat/Sun, Sat/Sun/Mon, etc)

As a Home Health Physical Therapist, you will:

• Confer with physician and clinical team members to obtain patient information and help develop, implement and revise the therapy treatment program

• Plan and administer prescribed skilled physical therapy treatment and training for patients suffering from various injuries, illnesses and functional disabilities to attain highest level of physical function

• Submit evaluation, treatment plans, progress reports and discharge summary to the supervisor and care management staff

• Provide Physical Therapy Assistants and Home Health Aide staff with written instructions and care plan that reflects current plan of care as related to therapy

• Oversee Physical Therapy Assistant and Home Health Aide staff performance, including monitoring the appropriate completion of documentation

• Report patient progress to the patient's physician, Clinical Manager, staff, patient and family

Required Qualifications

• Current and unrestricted Physical Therapy license in the state of North Carolina

• A valid driver's license, auto insurance, and reliable transportation

• Current CPR certification

Pay Range

$59.00 - $83.00 pay per visit/unit

$93,000 - $128,000 per year base pay

Additional Information

Driving Statement

You will be a part of Humana's driver safety program and therefore will need a valid state driver's license and proof of personal vehicle liability insurance with at least 100/300/100 limits.

TB Statement

You will be patient facing, therefore apart of Humana's Tuberculosis (TB) screening program. If selected for this role, we will require you to undergo TB screening.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
