Home Health Physical Therapist (PT)

Schedule: Monday–Friday, 8:00 AM–5:00 PM

We're seeking a dedicated and compassionate Home Health Physical Therapist (PT) to provide exceptional rehabilitative care to patients in the comfort of their homes. Position Details

• Schedule: Monday–Friday, 8:00 AM–5:00 PM
• Productivity Requirement: 30 points per week
• Mileage Reimbursement: $0.43 per mile
• License Required: Active Physical Therapist (PT) license

Coverage Area

This position serves patients in the following Alabama counties:

• Butler
• Crenshaw
• Lowndes
• Covington

Responsibilities

• Perform comprehensive physical therapy evaluations based on physician referrals.
• Develop individualized treatment plans with measurable goals to maximize patient function and independence.
• Provide skilled physical therapy services in patients' homes.
• Regularly assess patient progress and modify treatment plans as needed.
• Educate patients and caregivers on therapeutic exercises, mobility techniques, fall prevention, and home safety.
• Collaborate with physicians, nurses, case managers, and other healthcare professionals to coordinate patient care.
• Recommend and train patients in the use of adaptive equipment to improve safety and functional independence.
• Complete accurate and timely clinical documentation, evaluations, treatment notes, and billing records.
• Maintain compliance with HIPAA, company policies, and all applicable state and federal regulations.
• Supervise Physical Therapist Assistants (PTAs) and Physical Therapy Aides in accordance with state practice guidelines.
• Participate in staff meetings, continuing education, and professional development activities.

Required Qualifications

• Active Physical Therapist (PT) license.
• Home Health experience required.
• HomeCare HomeBase (HCHB) experience preferred but not required.
• Strong clinical assessment, treatment planning, and documentation skills.
• Excellent communication and patient education abilities.
• Valid driver's license, reliable transportation, and current auto insurance.
• Ability to work independently while managing a designated patient territory.
