RN Patient Care Manager

Hospice Nurse Practitioner
Roles & Responsibilities:
- Review and assess incoming patient referrals to determine care needs and assign appropriate hospice personnel
- Evaluate patient diagnoses, prognosis, medications, and overall clinical status
- Support clinicians in developing and implementing individualized plans of care
- Facilitate coordination and communication among interdisciplinary care teams
- Conduct periodic medical record reviews and report findings to leadership
- Supervise hospice clinical staff, including participation in hiring, onboarding, and training
- Assist in organizing and coordinating ongoing education and development programs for staff
- Participate in quality assurance and performance improvement initiatives
- Ensure compliance with all applicable state and federal regulations
- Perform additional duties as assigned
Qualifications & Certifications:
- Active Registered Nurse (RN) license in the state of practice
- Bachelor of Science in Nursing (BSN) preferred
- Prior experience in hospice or home healthcare preferred
- Minimum 2+ years of supervisory or management experience
- Strong clinical assessment, decision-making, and communication skills
- Knowledge of hospice care principles and pain/symptom management
- Commitment to high standards of clinical care and documentation
