Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

This opportunity is seeking a full-time Medication Therapy Management (MTM) pharmacist to support specialty MTM needs. The position offers a 13-week contract with potential for direct hire. Work onsite or remotely with a requirement to maintain a Louisiana pharmacist license plus at least five additional licenses from a designated list of states, including DC, North Dakota, New Hampshire, New Jersey, South Carolina, South Dakota, Utah, and Wyoming.

Key Responsibilities:

• Prepare and dispense medication orders as per physician requests, following established protocols.
• Interpret both verbal and written prescriptions and accurately update computerized medication profiles.
• Maintain comprehensive and accurate pharmacy patient medication profiles.
• Dispense pharmaceuticals and controlled substances in compliance with legal requirements.
• Monitor drug regimens for contraindications, interactions, allergies, and appropriate dosing.
• Detect and report adverse drug reactions promptly.
• Provide clinical consultation and suggest cost-effective therapeutic alternatives to medical staff.
• Document all clinical interventions thoroughly.
• Participate in quality improvement and medication use review activities, including data collection and inspections.
• Supervise pharmacy support personnel and verify pharmacy technician activities.
• Organize and prioritize work to ensure timely pharmacy services with minimal supervision.
• Conduct medication storage area inspections and maintain drug inventory integrity.
• Professionally handle telephone and patient interactions at the window.

Qualifications:

• Current Louisiana pharmacist license required.
• Licenses in at least five additional states from the specified list.
• Prior retail or hospital pharmacy experience essential.
• Demonstrated ability to obtain a minimum of 10 additional state licenses beyond Louisiana within 18 months in this role.
