Weekend General Inpatient Nurse - (RN) Case Manager - Beaumont Hospice

We CARE for our patients like they are our own FAMILY. Schedule 8- or 10-hour shifts. Shift options: 8a-6p, 9a-7p, 9a-5p, or 10a-6p. Every other weekend (Saturday and Sunday). 1-2 weekdays on the week opposite of working weekend; 1 day throughout the week (11:00am – 7:00pm). Works during the week variation; open availability required. One summer and one winter holiday required. Orientation: Week 1 M-F 9am-4pm and 3 days per week, including weekend rotation thereafter. Location & Agency Agency: Beaumont Hospice. Territory: Corewell Troy Hospital. Key Responsibilities Provide end-of-life care and Hospice services in the Health System Inpatient Hospice setting. Assist in determining the need to change the level of care based on assessment and collaboration with the Attending Physician or Medical Director. Implement care plan to achieve comfortable and safe dying, self-determined life closure, and effective grieving. Serve as Family Educator/Advocate to maintain dignity and support spiritual and cultural beliefs. Complete clinical documentation following agency protocol and Medicare/Federal guidelines. Provide referrals and coordinate transition from hospital to hospice care. Attend interdisciplinary team meetings for patient/family needs. Hard & Soft Skills Compassionate communicator with a positive attitude. Patience when working with patients, families, physicians, and coworkers. Independent worker with demonstrated skills in patient care and education. Attention to detail and ability to follow directions. Qualifications Registered Nurse within the state of employment. Bachelor’s Degree in Nursing preferred. Minimum 3 years inpatient clinical experience, preferably hospice. Valid driver’s license and auto insurance in your name as a driver.
