Nurse Practitioner for In-home Visits

Job Overview Signify Health, a division of CVS Health®, is hiring board‑certified nurse practitioner contractors to conduct in‑home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. Responsibilities Conduct in‑home visits focused on a member’s overall health and well‑being and identify potential barriers to care. Complete medication and medical history review and perform a physical evaluation in the member’s home. Use pre‑populated, patient‑specific forms to document each evaluation. Compensation & Schedule Earn up to $2,500 to $4,000 each week (average 6–9 visits daily, base rate $80 per completed assessment). Increase per‑visit rate up to 135% or 160% with the Market Multiplier (depending on working counties). Earn additional compensation via a pay‑per‑test model for qualified visits. Work Monday to Sunday, with no nights or weekends required. Short‑term or long‑term contracts available. Independent contractor (1099) with flexible scheduling. Benefits Airfare, hotel, and rental car coverage for eligible trips. Malpractice coverage. Availability incentives based on weekly availability. Qualifications Board‑certified nurse practitioner.
