The Care Coordinator is responsible for clinical assessments, development of recovery plans in collaboration with the treatment team and provision of services based on consumer needs utilizing Trauma Informed and Patient Centered principles, and compliance with performance contract requirements. This role includes coordinating, linking and monitoring appropriate services, as well as assessing consumer progress towards recovery goals. Delivers interventions such as skills training, psychosocial rehabilitative services, case management, care coordination, and medication training/monitoring and supports. Care Coordinators are community-based positions which means the person must be able to travel to consumer homes or other field locations when providing services (as needed). Unit specific needs are determined by unit leadership including the portion of each day that may be allotted for clinic-based versus community-based service provision
Your Role in Action- Delivery of Skills Training, Psychosocial Rehabilitation Service, and Medication Training & Support via Evidence-Based Practices that are trauma informed and person-centered to support patients in their recovery to address the severe and persistent mental illness and symptom related problems that interfere with the individual's functioning.
• Connecting people served via Case Management/Care Coordination to community resources, including Medicaid supported programs. Emphasis is on connecting patients to medical providers and establishing patients with available benefits based on individual needs.
• Ensures all services provided meet eligibility criteria, medical necessity, and are authorized in accordance with agency and regulatory guidelines.
• Attends unit training, team meetings, and staffing of patients with Treatment team.
• Participating in clinical assessments and formulation of recovery plans based on needs and assures recovery plan is authorized with a CTL/LPHA signature for consumers on caseload. - Provides interventions in response to crisis to reduce symptoms and avoid admission to a more
restrictive environment.
• Provides training/education on the nature of mental illness, the importance of medications, and other
medication related information as outlined in IMR/Harris Center curriculum.
• Monitors consumer progress with treatment goals and compliance with eligibility requirements for
ongoing treatment (CANS/ ANSA, financial assessment, current diagnosis) using the monthly SAI report
and other available information.
• Comply with agency policies and procedures including Standards of Behavior; accrediting/regulatory
bodies; performance contract documentation requirements; and unit specific guidelines.
• Complete all required training (e.g., all evidence-based practices, trauma informed and person
centered training) by expected due dates upon hire and as assigned thereafter.
• Travel and Transportation of patients (as required by the unit), including but not limited to medication
delivery, medical appointments, meetings to support patient Medicare and Medicaid benefits, and
other patient community resources. (0% for CAS).
• Adapts to changing work requirements and works independently to complete assignments by
deadlines.
• Performs other related duties as assigned.
- Bachelor's degree (B.A.) from four-year accredited college or university with a major in a social, behavioral, or human service field, including but not limited to: psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, and educational psychology, required.
- 1 year working in the field of mental health preferred.
- Must maintain a valid Texas Driver's License and be insurable under Agency policy.
- The ideal candidate will be required to transport consumers using their personal vehicle and will be reimbursed for mileage by the Agency.
- Basic understanding of mental illness, common signs and symptoms and able to identify difficulties with coping in community settings.
- Strong communication skills, listening and interpersonal skills above average, written documentation is clear, concise, well- constructed and grammatically correct.
- Able to work collaboratively with others in a teamwork environment.
- Effective time management and organizational skills. Able to execute standard word processing.
- Must be able to work autonomously in a fast-paced environment.
