Housing & Health Access Case Manager

3M HEALTHCARE

The Colorado Coalition for the Homeless is seeking a Case Manager I to work collaboratively with case managers, supervisors, health team members, and community partners to promote long-term solutions for families, children, and individuals experiencing homelessness. You will develop care plans, coordinate services, monitor progress, advocate for clients, and maintain thorough case records.
