Physical Therapist (PT) - Home Health

We are seeking a dedicated and experienced Physical Therapist to join our Home Health team. The ideal candidate will provide exceptional rehabilitation services to patients in their homes, promoting independence and improving their quality of life.

Key Responsibilities

• Conduct comprehensive assessments to evaluate patients' physical needs and develop individualized treatment plans.
• Implement therapeutic interventions aimed at improving mobility, strength, and overall function.
• Educate patients and their families on proper exercises, strategies for injury prevention, and rehabilitation techniques.
• Collaborate with healthcare professionals to ensure coordinated and effective patient care.
• Document patient progress accurately and timely, adjusting treatment plans as necessary.
• Provide ongoing support and motivation to patients throughout their rehabilitation journey.

Qualifications

• Licensed Physical Therapist in the state of practice.
• Experience in home health care and proficiency with OASIS documentation is required.
• Strong communication and interpersonal skills.
• Ability to work independently and manage a varied caseload.
• Valid driver's license and reliable transportation for travel to patient locations.

Work Schedule

The position offers a day shift, working 5 days a week, for 8-hour shifts from 08:00 to 16:30.

Additional Information

Mileage reimbursement will be provided at the IRS rate.
