Director of Nursing
TalentoHC
Director of Nursing
We are seeking a modern clinical operations leader who combines strong nursing leadership with technology, accountability, and performance management.
This individual will oversee clinical operations and field-based care professionals, maintain high standards for patient care and regulatory compliance, lead quality assurance and performance improvement initiatives, and ensure ongoing readiness for regulatory reviews.
The role requires hands-on proficiency with electronic clinical documentation and workforce management systems, data-driven performance monitoring, and the ability to build a clinical team that consistently meets documentation, productivity, and quality expectations.
This position also includes designated backup administrative responsibilities. The successful candidate must be capable of learning and overseeing the organization's clinical, administrative, compliance, intake, billing, and personnel functions to provide leadership coverage when needed.
Required background:
Candidates must have experience working in home-based healthcare services and prior management experience overseeing field-based clinical personnel within a regulated, publicly funded care environment. Experience limited to hospitals, residential facilities, or other care settings without qualifying home-based care management experience will not meet the requirements.
Key Responsibilities
Clinical Leadership and Team Management
- Lead clinical operations and serve as the senior clinical authority.
- Supervise clinical managers, care coordinators, liaison personnel, and field-based care professionals.
- Manage staff assignments, clinical capacity, and continuity of care.
- Hold field personnel accountable for completing visits, submitting documentation promptly, and complying with timekeeping and visit-verification requirements.
- Conduct and document supervisory visits and competency evaluations.
Documentation, Timekeeping, and Reporting
- Ensure clinical records are complete, accurate, and submitted on time, including assessments, care plans, service records, orders, and supervisory documentation.
- Ensure personnel follow established clock-in, clock-out, and visit-verification procedures using approved company systems.
- Review timekeeping and service records weekly, promptly address missing entries or late documentation, and implement corrective action for recurring issues.
- Ensure all required clinical reports are submitted on schedule and provide management with timely performance reports.
Administrative Leadership and Business Continuity
- Serve as the designated backup for administrative leadership when the primary administrator is unavailable.
- Develop working knowledge of compliance, licensing, policies, regulatory readiness, intake, referrals, billing, authorizations, personnel, credentialing, and office operations.
- Work closely with senior management to document and transfer operational knowledge.
- Maintain readiness to assume administrative leadership responsibilities when required.
Admissions and Care Coordination
- Review and support new admissions and coordinate service initiation with intake personnel.
- Monitor the timeliness of initial assessments, resumption of services, recertification, and discharge.
- Ensure care plans and required clinical orders are accurate, authorized, and maintained in the record.
- Oversee the accuracy and timely completion of required standardized clinical assessments.
Quality Assurance and Regulatory Compliance
- Maintain compliance with applicable federal and state requirements, professional standards, and organizational policies.
- Lead quality assurance and performance improvement activities, including data analysis, improvement projects, and documented review meetings.
- Oversee infection prevention, clinical competencies, and staff education.
- Monitor patient outcomes, including acute-care utilization, and implement improvements based on identified trends.
- Investigate clinical incidents and complaints and oversee corrective action.
Additional Service-Line Oversight
- Provide clinical oversight for supplemental care services offered by the organization.
- Ensure associated documentation, reporting, and timekeeping requirements are met.
- Apply consistent quality and accountability standards across all service lines.
Survey Readiness and Performance Management
- Maintain ongoing readiness for regulatory inspections and record reviews.
- Lead clinical preparation for external reviews and oversee corrective action plans.
- Develop and maintain performance dashboards covering documentation timeliness, assessment accuracy, visit completion, productivity, and patient outcomes.
- Use performance data to manage accountability and communicate results to senior leadership.
Technology and Electronic Workflows
- Independently use the organization's electronic systems for clinical oversight, review, and approval.
- Establish efficient electronic workflows that ensure information is maintained in approved company systems rather than informal or personal records.
- Promote consistent use of technology across clinical teams.
Growth and Partnership Support
- Support service growth by ensuring the organization can assess, accept, and staff appropriate referrals efficiently.
- Coordinate with business development and referral coordination personnel on clinical questions and service capabilities.
- Mentor and develop clinical personnel and support succession planning.
External Representation and Relationship Management
- Represent the organization at community events and partner meetings, including occasional evening or weekend commitments.
- Participate in meetings with referral partners and other care organizations as the senior clinical representative.
- Clearly communicate the organization's approach to patient care, monitoring, quality, and reporting.
- Build confidence among professional partners, families, and community stakeholders.
- Collaborate with outreach and relationship-management personnel to support events and strengthen partnerships.
Qualifications
Required
- At least two years of experience in home-based healthcare services.
- At least two years in a clinical management or supervisory role within a regulated home-based care organization, with direct responsibility for field-based clinical personnel. The experience requirements may overlap.
- Active, unrestricted professional nursing license appropriate to the position and jurisdiction.
- Graduation from an approved nursing education program.
- Meets applicable regulatory requirements for the clinical leadership position and designated backup administrative responsibilities.
- Strong working knowledge of applicable care-delivery regulations, standardized clinical assessments, reimbursement frameworks, and quality improvement processes.
- Demonstrated experience supporting successful regulatory surveys or inspections, including specific examples of resolving deficiencies.
- Proven ability to manage field personnel accountability, documentation quality, and service-completion requirements.
- Hands-on proficiency with electronic clinical record systems and clinical review workflows.
- Ability to satisfy required background screening and applicable exclusion checks.
- Willingness to work as a direct, full-time employee of the organization.
Preferred
- Previous experience in administrative leadership or designated backup administration within a regulated home-based care organization.
- Bachelor's or master's degree in nursing.
- Relevant certifications in clinical assessment, documentation, or healthcare coding.
- Experience with electronic clinical documentation and workforce management platforms.
- Experience supporting service growth, referral relationships, and intake operations.
- Bilingual communication skills.
Regulatory and Leadership Requirements
The successful candidate must meet all applicable professional licensing, supervisory experience, employment, background screening, and clinical oversight requirements for the position.
The individual must be qualified for the organization's designated clinical leadership responsibilities and any formally assigned administrative coverage duties. Required designations, supporting documentation, and regulatory filings must be maintained in accordance with applicable law.
The role includes oversight of patient care services and personnel, staff assignments, care coordination, ongoing assessment of patient needs, and the development, implementation, and updating of individualized care plans.
Why Relevant Industry Experience Matters
Prior experience in home-based care management is essential because the position requires immediate familiarity with field personnel oversight, standardized assessments, reimbursement requirements, regulatory surveys, documentation standards, and care coordination.
The organization seeks a leader who can assume responsibility promptly, strengthen existing systems, improve clinical performance, and maintain consistent operational standards without an extended introduction to the care setting.
