Hopedale Medical Complex is seeking a dedicated Certified Nursing Assistant (CNA) to join our Nursing Home team. In this role, you will provide hands-on care to residents while working alongside an experienced interdisciplinary team committed to quality, compassionate, and resident-centered care. You will assist residents with activities of daily living, monitor and report changes in condition, and help maintain a safe, comfortable, and respectful environment. This position is primarily based in the HMC Nursing Home , with opportunities for cross-training and occasional floating to the Hospital/Med Surg unit based on patient care needs, staffing, and training. Schedule Full-Time Day Shift: 6:00 a.m.–6:30 p.m. Rotating weekends and holidays required Schedule may vary based on departmental and resident care needs Essential Duties and Responsibilities Resident Care Provide compassionate, high-quality direct resident care under the supervision of an RN or LPN. Assist residents with activities of daily living, including bathing, dressing, grooming, toileting, mobility, and meals. Assist residents with positioning, transfers, ambulation, and other mobility needs while maintaining safety and comfort. Follow individualized care plans and nursing instructions. Assist with maintaining resident comfort, dignity, privacy, and independence. Observe residents and promptly report changes in condition, behavior, or other concerns to licensed nursing staff. Assist with maintaining a clean, safe, and comfortable resident environment. Provide care in accordance with established infection prevention and safety practices. Teamwork and Communication Communicate effectively with residents, families, nursing staff, and other members of the interdisciplinary care team. Work collaboratively with RNs, LPNs, and other healthcare professionals to support quality resident care. Demonstrate professionalism, dependability, compassion, and respect in interactions with residents, families, and coworkers. Respond appropriately to resident needs and changes in condition. Maintain resident confidentiality and comply with HIPAA requirements. Professional Responsibilities Maintain compliance with HMC policies, procedures, and resident safety standards. Participate in required in-service training, continuing education, competency assessments, and other professional development activities. Demonstrate a commitment to quality improvement and resident-centered care. Maintain current knowledge and skills necessary to perform assigned duties safely and effectively. Cross-train and float to other HMC departments or facilities as assigned. Perform other duties as assigned. Required Qualifications High school diploma or equivalent required. Current Illinois Certified Nursing Assistant (CNA) certification required. Current CPR certification required. Excellent communication and customer service skills. Compassionate and respectful approach to resident care. Ability to work effectively as part of an interdisciplinary healthcare team. Ability to follow nursing instructions, individualized care plans, and established policies and procedures. Strong attention to detail and ability to recognize and report changes in resident condition. Ability to maintain resident confidentiality and comply with HIPAA requirements. Ability to perform the essential functions of the position with or without reasonable accommodation. Willingness to cross-train and float to other Hopedale Medical Complex departments and facilities as needed. Preferred Qualifications Previous experience working in a nursing home, long-term care, assisted living, hospital, or other healthcare setting. Experience providing direct care to older adults or individuals with complex healthcare needs. Previous experience working as part of an interdisciplinary healthcare team.
