Primary Care Physician (Geriatrics) - Multiple San Diego Locations - Senior Living Facilities

Position: Full-Time Primary Care Physician (Geriatrics) - Multiple San Diego Locations - Senior Living Facilities (Part Time may be an option) Job Type: Full-Time Schedule: Monday to Friday (No Weekends) OR Part-Time (No Weekends) Location: San Diego Area About Us: Aligned Geriatrics is a rapidly growing mobile medical practice that specializes in Geriatric Medicine. Our mission is to provide high-quality and compassionate care for seniors in the comfort of their homes or senior living communities. As a primary care provider with Aligned Geriatrics you will play a crucial role in improving the lives of older adults by delivering personalized healthcare services. Physician Responsibilities: Conduct regular rounding at assisted living and memory care facilities, managing both acute and chronic conditions. Run multiple in-house clinics at senior living communities with set hours each week seeing independent, assisted living and memory care patients (Patients are pre-scheduled). Complete history and physicals on new admissions and oversee care transitions. Respond to urgent medical issues during facility hours. Collaborate with nursing, therapy, and administrative staff to ensure coordinated care. Communicate with families and responsible parties regarding treatment plans and changes. Document encounters and medical decision-making clearly and timely in the EMR. Participate in quality assurance and performance improvement activities. Follow all federal, state, and facility-specific regulations and protocols. Qualifications: Medical Doctor (MD) or Doctor of Osteopathy (DO) degree Board-certified or board-eligible in Internal Medicine, Family Medicine, or Geriatric Medicine (preferred) Active and unrestricted California medical license Experience in Geriatric Medicine and/or primary care is a plus Strong communication and interpersonal skills Experience with Epic (EHR) is a plus Experience with Microsoft Office is a plus Strong time management and organizational skills Comfortable working independently and in team based environments Compassionate bedside manner with a commitment to providing patient centered care. Experience in skilled nursing or post-acute care preferred. Strong communication and clinical decision-making skills. Ability to work independently while collaborating effectively with facility staff. Schedule & Compensation: Competitive compensation based on experience.
