Care Coordinator

Humana is seeking a Care Coordinator to support members across Healthcare & Medical Services. In this role, you will coordinate care, close gaps in services, and connect members with providers and community resources to improve health outcomes. You’ll conduct outreach, assessments, and education while tracking care plans in electronic systems. Working within Humana’s member-focused, inclusive culture, you’ll collaborate with clinicians and partners to simplify healthcare, support well-being, and deliver personalized, high-quality experiences for diverse populations.
