Social Services
Brookside Healthcare and Rehabilitation Center, a 120-bed beautiful facility, recognizes and respects that Brookside is home to our residents. Therefore, we take a personalized approach in caring for our residents, providing comprehensive medical, emotional, and social support. We are searching for experienced, caring, & dependable applicants to join our team.

Responsibilities:
-Assures that notes are informative and descriptive of services provided and of residents’ responses to services
-Prepares and completes social service documentation promptly
-Develop a social history on each resident
-Complete sections of the MDS
-Assists with addressing resident concerns and searching for a positive resolution
-At times, participates in the Utilization Review and other committees as directed and needed
-Develops discharge plans at the point of admission to provide direction for the resident's stay
Qualifications:
-BSW or MSW
-Knowledgeable of social services practices, procedures, laws, regulations, and guidelines governing long-term care
-Previous long-term care experience is desirable. Past supervisory experience preferred.
