Job Description:\n\nDescription:\nPosition Summary\nThe Remote Patient Monitoring (RPM) Nurse / Program Coordinator is responsible for the day-to-day operation, coordination, monitoring, and ongoing development of Kane County Hospital’s Remote Patient Monitoring and Digital Care Program.\nThis position serves as the primary operational lead for the RPM program and works collaboratively with Kane County Hospital providers, clinic staff, Information Technology, billing and finance personnel, and technology partners to ensure the program is implemented effectively and operates safely, efficiently, and consistently.\nThe RPM Nurse / Program Coordinator is expected to take initiative, identify needs and barriers, develop solutions, follow issues through to resolution, and maintain ownership of program operations. The position requires a highly organized, self-directed nurse who is comfortable building new workflows, learning new technology, communicating with patients and providers, tracking performance, and independently managing competing priorities.\nThe Operations Director will provide program oversight. ChartSpan and other implementation partners will provide technical assistance, education, and guidance as the program is established and refined.\n\nEssential Duties and Responsibilities\nRPM Program Management\nCoordinate and manage the day-to-day operations of the Remote Patient\nMonitoring Program.\nServe as the primary internal point of contact for RPM program questions,\nworkflows, implementation needs, and operational concerns.\nTake ownership of program implementation and ongoing operations with oversight\nfrom the Operations Director.\nWork with ChartSpan and other technology partners during implementation,\ntraining, troubleshooting, and workflow development.\nDevelop, maintain, and improve standardized RPM workflows, procedures, and\noperational processes.\nIdentify workflow problems or program barriers and proactively develop solutions.\nFollow identified issues through resolution rather than waiting for direction on\nroutine operational matters.\nMaintain organized records related to program enrollment, devices, monitoring,\nclinical interventions, performance measures, and required reporting.\nCoordinate meetings and communication among providers, clinic staff, technology\npartners, IT, billing, and hospital leadership as needed.\nKeep the Operations Director informed of program progress, significant concerns,\nbarriers, and opportunities for improvement.\n\nPatient Identification and Enrollment\nWork with providers and clinic staff to identify patients who may benefit from\nRemote Patient Monitoring.\nInitially support RPM enrollment for patients with conditions such as hypertension\nand diabetes, with expansion to additional populations as the program develops.\nSupport future expansion to patients with congestive heart failure, COPD, postdischarge\nmonitoring needs, and other eligible conditions as approved.\nReview referrals and verify that patients meet established program eligibility\nrequirements.\nContact eligible patients and explain the RPM program.\nObtain and document appropriate patient consent.\nEnroll patients into the RPM platform according to established procedures.\nCoordinate distribution and activation of monitoring devices.\nEducate patients and caregivers on device use, monitoring expectations,\ncommunication procedures, and when to seek urgent or emergency care.\nProvide additional support for patients with limited health literacy, digital literacy,\ntransportation access, or technology experience.\n\nPatient Monitoring and Clinical Oversight\nRoutinely review patient-generated health information received through the RPM\nplatform according to established monitoring standards.\nMonitor data such as blood pressure, blood glucose, weight, oxygen saturation, and other approved measurements based on the patient's condition and care plan.\nIdentify readings, trends, symptoms, or changes that fall outside established\nclinical parameters.\nContact patients as appropriate to obtain additional information and complete\nnursing assessment within the RN scope of practice.\nProvide patient education and reinforce established care plans.\nDocument monitoring activities, patient communication, interventions, and\nescalation in accordance with KCH standards.\nPromptly communicate clinically significant findings to the appropriate provider.\nFollow established escalation protocols for abnormal readings or concerning\npatient symptoms.\nTrack provider recommendations and assist with ensuring appropriate follow-up\noccurs.\nRecognize situations requiring urgent or emergency evaluation and direct patients\naccording to established clinical protocols.\nMaintain clear boundaries between nursing monitoring and provider medical\ndecision-making. Providers remain responsible for diagnosis, treatment changes,\nprescribing, and other medical decisions.\n\nProvider Communication and Follow-Up\nServe as a communication link between RPM patients and their healthcare\nproviders.\nProvide providers with concise, relevant information regarding abnormal readings,\npatient concerns, and meaningful trends.\nTrack escalated concerns to ensure appropriate follow-up has occurred.\nAssist with coordinating follow-up appointments, lab work, imaging, medication\nreview, or other services ordered by the provider.\nCommunicate recurring trends or concerns that may warrant adjustment of the patient's monitoring plan.\nWork collaboratively with Medical Assistants, nurses, registration staff, and other\nclinical personnel to prevent gaps in patient follow-up.\n\nDevice and Technology Coordination\nMaintain knowledge of the RPM platform and approved monitoring equipment.\nCoordinate device assignment, distribution, activation, replacement, and return.\nMaintain accurate device inventory and patient assignment records.\nAssist patients with basic troubleshooting and coordinate higher-level technical\nsupport through ChartSpan or other technology partners when needed.\nMonitor unsuccessful or missed data transmissions and follow up according to\nestablished procedures.\nWork with IT and technology partners to identify and resolve system or connectivity\nconcerns.\nParticipate in testing and optimization of Cerner CommunityWorks integration and\nRPM documentation workflows.\n\nProgram Tracking and Reporting\nThe RPM Nurse / Program Coordinator will maintain accurate program data and routinely\ntrack measures including:\nPatients identified for RPM\nProvider referrals\nPatients contacted\nEnrollment acceptance and decline rates\nActive patient enrollment\nDevice activation\nSuccessful data transmission\nMissed transmissions\nPatient retention\nMonitoring activity\nAbnormal readings and escalations\nClinical interventions\nCompleted follow-up\nDevice utilization\nTechnical support needs\nPatient satisfaction\nProgram barriers\nEmergency department visits, hospitalizations, or readmissions when applicable\nOther required grant and quality measures\nThe RPM Nurse / Program Coordinator will assist the Operations Director and other KCH\nleadership with preparation of required program reports and performance information.\n\nGrant and Quality Responsibilities\nSupport implementation of the LIFT Initiative Remote Patient Monitoring and Digital\nCare project.\nAssist with collection of required grant performance measures and supporting\ndocumentation.\nMaintain accurate records to support program monitoring, audits, and grant\nreporting.\nParticipate in quality-improvement activities related to the RPM program.\nCompare program performance with established baseline measures and project\ngoals.\nIdentify trends, gaps, and opportunities for program improvement.\nHelp evaluate whether the RPM program is reaching patients with significant rural\naccess barriers.\nParticipate in program reviews with hospital leadership and implementation\npartners.\nAssist with preparing for grant monitoring activities or site visits as requested.\n\nBilling and Sustainability Support\nWork with billing, finance, providers, and leadership to establish documentation\nworkflows that support compliant RPM billing.\nEnsure required clinical activities and patient interactions are appropriately\ndocumented.\nAssist with identifying operational issues that could affect billing or reimbursement.\nParticipate in monitoring patient participation, program utilization, and operational\nefficiency.\n-term goal of integrating RPM into routine chronic\ndisease management services.\nAssist leadership in evaluating opportunities to expand the program as patient\nneeds, reimbursement, and organizational capacity evolve.\n\nStaff Training and Program Education\nprogram.\nParticipate in implementation and platform training provided by ChartSpan and\nother technology partners.\nAssist with training providers, Medical Assistants, nurses, registration personnel,\nand other staff involved in RPM workflows.\nProvide ongoing education as workflows change or new patient populations are\nadded.\nDevelop or maintain patient education materials and staff reference resources.\nReinforce consistent use of approved workflows, documentation standards, and\nescalation procedures.\n\nInitiative and Program Ownership\nThe individual in this position is expected to function with a high level of independence and\ninitiative.\nThe RPM Nurse / Program Coordinator is expected to:\nRecognize what needs to be completed and take appropriate action without\nrequiring continuous direction.\nIndependently organize and prioritize daily program responsibilities.\nFollow through on outstanding issues until resolution.\nSeek answers and utilize available resources when unfamiliar situations arise.\nCommunicate directly with ChartSpan and other approved partners when\nimplementation or technical guidance is needed.\nIdentify problems before they become significant barriers.\nBring appropriate clinical, operational, financial, or safety concerns to the attention\nof the Operations Director or provider.\nRecommend improvements rather than simply identifying problems.\nAdapt as the program evolves.\nMaintain accountability for deadlines, patient follow-up, documentation, program\ntracking, and assigned responsibilities.Requirements:\nQualifications\nRequired:\nCurrent Registered Nurse license in the State of Utah or compact-state eligibility as\napplicable.\nCurrent BLS certification.\nStrong clinical assessment and communication skills.\nStrong organizational skills and attention to detail.\nAbility to work independently and manage multiple priorities.\nDemonstrated initiative, dependability, and follow-through.\nAbility to communicate professionally with patients, providers, leadership, and\noutside partners.\nAbility to learn and effectively use electronic health records, remote monitoring\nplatforms, and other healthcare technology.\nAbility to recognize changes in patient condition and appropriately escalate clinical\nconcerns.\n\nPreferred:\nExperience in outpatient nursing, chronic disease management, care coordination,\npopulation health, case management, or remote patient monitoring.\nExperience caring for patients with hypertension, diabetes, CHF, or COPD.\nExperience with Cerner CommunityWorks or similar electronic health record\nsystems.\nExperience implementing a new clinical program or workflow.\nExperience with quality improvement, data tracking, grant-supported programs, or\nhealthcare program coordination.\n\nDesired Characteristics\nThe successful candidate should while also independently seeking solutions\n\nReporting Structure\nThe RPM Nurse / Program Coordinator reports to the Operations Director, who provides\noverall administrative and operational oversight of the program.\nThe RPM Nurse / Program Coordinator works collaboratively with KCH providers regarding\nclinical patient-care decisions. The nurse may identify, assess, document, and escalate\nconcerns within the RN scope of practice; however, providers remain responsible for\nmedical diagnosis, medication changes, treatment decisions, and determination of higher\nlevels of care.\nChartSpan and other approved implementation partners will serve as resources for\nimplementation guidance, platform support, workflow development, training, and\ntroubleshooting as appropriate.\n\nPerformance Expectations\nPerformance will be evaluated in areas including:\nTimeliness and accuracy of patient monitoring\nAppropriate escalation of clinical concerns\nPatient enrollment and retention\nCompletion of patient follow-up\nAccuracy and completeness of documentation\nProgram organization and reliability\nDevice and patient tracking\nResponsiveness to patients and providers\nCompliance with established workflows\nGrant data collection and reporting\nIdentification and resolution of program barriers\nSuccessful collaboration with technology partners\nInitiative and independent problem-solving\nContribution to program growth and sustainability\nPatient and provider satisfaction
