Contract (13 Weeks) | Full-Time | Days | Monday – Friday, 8:00 AM – 4:30 PM | Onsite

Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting
• Manage a caseload of approximately 28 points per week
• Document patient care and progress using HCHB (Homecare Homebase)
• Complete a minimum of 2 days of in-office orientation during the first two weeks
• Participate in optional weekend on-call rotation, approximately once per month

Required Qualifications

• Active Physical Therapist (PT) license in the state of Connecticut
• Prior home health experience required
• Proficiency with HCHB (Homecare Homebase) documentation system required
• Must reside within 40 miles of the Watertown, CT office to be eligible

Preferred Experience / Skills

• Experience managing productivity-based caseloads in a home health environment
• Familiarity with home health regulatory and compliance standards
