Shift Details
Monday-Friday, 8:00 a.m. - 5:00 p.m.
Requirements
Board Certified in Internal Medicine
Active Arizona license or IMLC with LOQ
Must have experience working with older adults, typically those over 65 years of age
Clean malpractice and background
Active DEA - Highly Preferred
Fellowship training in aging - Highly Preferred
Assignment Details
Patient population: Geriatric
Average admissions: 6-12
Setting: Clinic, no trauma, not rated
EMR: Cerner
Support staff: Medical assistant, registered nurse
Job duties include psychosocial and emotional support, goals of care facilitation, family counseling and education, interdisciplinary collaboration, and non-clinical care management
Focus on managing complex chronic conditions, preventing age-related disabilities, and coordinating care to maximize patient independence and quality of life
