Requirements:

• Valid LCSW license required
• BLS certification required
• Minimum of 1 year of experience
• Charting experience required (Epic)

Roles & Responsibilities:

• Conduct biopsychosocial assessments and participate in treatment planning.
• Coordinate discharge planning and collaborate with interdisciplinary teams.
• Provide referrals to community resources and benefits assistance.
• Handle protective services, guardianship, DCF navigation, and involuntary holds.
• Provide counseling to individuals, families, and couples, and participate in care conferences.

Compliance / Notes:

• Compliance documents due the Monday prior to the start date.
• Must provide the last four of SSN during compliance.
• Reference requirements: 2 covering 1 year within the last 3 years.
• Must declare RTO/scheduling requests at submission; post-submittal RTO likely not approved.
• Must complete pre-start modules, averaging 16 hours; if over 20 hours, PM approval is required.
