Job Title: Licensed Vocational Nurse/ Licensed Psychiatric Technician

Location: 1060 Howard Street, San Francisco, CA 94103

Schedule: Monday - Friday, 11am - 7pm

Reporting Type: In-Person

Employment Type: Non-Exempt, Full Time

Salary Range: $38.50/hour

Licensing Requirements: California (CA) Licensed Vocational Nurse (LVN) or Licensed Psychiatric Technician (LPT) license, Registered Nurse (RN) or higher - Must be current and in good standing.

Summary:

Swords to Plowshares Service Center currently provides community service programming for senior veterans and adult veterans with disabilities. Current services provided are as follows:


The Licensed Vocational Nurse/ Licensed Psychiatric Technician (LVN/LPT) is responsible for the delivery and coordination of medical and behavioral health case management services offered at our Veteran Community Center. The LVN/LPT at the Veteran Community Center plays a critical role in providing healthcare support to veterans in a community-based setting. Additionally, the LVN/LPT is responsible for delivering direct nursing care, assisting with case management, and collaborating with a multidisciplinary team to support the health and well-being of veteran clients.

Responsibilities:

• Timely data entry and record management
• Responsible for being present on the VCC floor and interacting with clients by checking in with them, building rapport, educating them on the group offerings and on the services provided by the LVN.
• Responsible for actively assessing client needs on the VCC floor and inviting the client to check in one-on-one for minor health matters.
• Maintain professional composure and boundaries with clients, guests, and Swords team members in every interaction.
• Facilitates a minimum of two wellness groups twice a week in Diet and Nutrition, Movement, Meditation Group or another group that falls within the health category.
• Submit a written plan for approval by the Program Manager showing a brief description of the group, target population, schedule and budget, prior to implementation.
• Submit progress notes on individual clients and input group attendance list in ECM as soon as the group concludes and not later than the end of the day.
• Attend and actively participate in weekly supervision and team meetings. Ensure that you have completed the tasks assigned to you the previous week and provide your Program Manager with a weekly progress update.
• Participate in monthly training with 1060 Howard staff.
• Complete annual performance review and engage in the process interactively with your Program Manager.
• Provides case management services related to medical and behavioral health
• Expert level understanding of VA Healthcare and obtaining and maintaining a list of specific contacts to connect veteran clients with at the San Francisco VA Medical Center, the San Francisco Downtown Clinic, and the Vet Center.
• Staying up to date on non-VA local medical facilities and clinics to include knowing their hours of operation, their staff, and their services to be able to confidently perform warm hand offs for the veteran's coordination of care
• Attend and actively participate in community outreach events and local standdowns as assigned by Program Manager.
• Assess and assist veterans with Activities of Daily Living (ADL) skills to include helping mobile and immobile clients with hygiene and clothing changes.
• Assisting veterans in obtaining public benefits (i.e Cal Fresh, Medi-Cal, Medicare,etc.)
• Assisting veterans in obtaining medical and mobility supplies
• De-escalation and crisis intervention support. Document all incidents that you were involved in using the organization's incident report form which should be completed no later than 24 hours after the incident.
• Attend all mandatory training and meetings
• Provides non-emergency medical support
• Ensures compliance with federal and state regulations for the delivery of community behavioral healthcare services.
• Performs other duties as assigned by Program Manager and Clinical Director of Non-Residential Services.

Required Qualifications:

Experience:

• At least one year of experience working in a community behavioral healthcare setting.
• Experience with populations impacted by mental health, substance dependency, and homelessness.
• Ability to effectively communicate in English with staff, clients, and external partners and stakeholders.

Education:

• California (CA) Licensed Vocational Nurse (LVN) or Licensed Psychiatric Technician (LPT) license - current and in good standing.
• CPR/First Aid certification or ability to obtain within 60 days of employment.
