Registered Nurse - Case Manager 

acute care hospital experience in both UM and Care Coordination (discharge planning) required
Shift: Day shift - 830am to 5pm -
CMs are responsible for UM and care coordination
CM needs to be on rotation for weekend coverage.

Requirements

1-2 years CM experience in acute setting with experience in Interqual reviews, CMS requirements.
