Job Summary The Psychiatric Nurse Practitioner is responsible for providing integrated consultation with patients, as well as with behavioral health and medical colleagues, and direct patient care including psychiatric diagnostic evaluation, psychiatric medication services, and other preventive, educational, and therapeutic behavioral health services. The position requires professional expertise and responsibility for ensuring services meet the needs of patients and are being provided in accordance with best practices, state and federal law, and CVCH policies and procedures.

Job Specific Competencies

1. Provides behavioral health clinical assessment, diagnosis, treatment plans, and ongoing psychiatric medication services for patients. a. Patient consultations may be scheduled, integrated, or provided through a local hospital setting. b. Interventions follow evidence-based practices. c. Provides timely reports according to policy and maintains appropriate documentation. d. Treatment goals are measurable. e. Informed consent and diagnoses are reviewed with the patient in a timely manner. f. Complex cases are reviewed with a consulting and/or peer psychiatric provider.

2. Reviews, initiates, and maintains psychiatric medications for patients until referred back to PCP or alternate provider. a. Evaluates the need for medications, initiates, reviews, renews, stops, and/or makes changes to psychotropic medications orders in accordance with established CVCH protocols and guidelines and state license. b. Reviews with patient side effects of medications being prescribed. c. Ongoing monitoring of patient's symptoms and medications until patient is referred off caseload. d. Orders and interprets lab tests for patients receiving psychotropic medications as appropriate. e. Provides consultation to CVCH Medical professionals concerning use of psychotropic medications and other behavioral health issues.

3. May assist in clinical supervision, shadowing of patient care, and oversight with student trainees, as requested. a. Participates in group supervision and didactic trainings. b. Provides in-service education to staff concerning psychiatric medications or related topics. c. Documents and discusses patient care issues and reports feedback to trainee's supervisor. d. Documents every clinical supervision with a trainee, as applicable. e. Contributes feedback for performance evaluations of trainees.

4. Coordinates client care as part of the Primary Care Medical Home Model to assist with improved communication and care. a. Coordination occurs internally with therapists, psychologists, PCPs, nurses, and other staff members involved with the patient. b. Coordination occurs with external providers as care dictates (e.g. inpatient facilities, neurology, specialty providers). c. Sends notes or other necessary documentation as appropriate.

5. May provide psychiatric oversight to Managed Care Organizations CVCH is contracted with. a. Assists with oversight of the patients care as part of the treatment team. b. Makes psychotropic medication recommendations when consulting with treatment providers. c. Complex patients should be seen prior to any recommendations for medications, which is left to the discretion of the treating provider. d. Makes recommendations related to care and medications for ongoing management. e. Participates in any consultation groups related to care. f. Monitors and tracks patients according to guidelines.

6. Responsible for documentation according to organizational, federal, and state guidelines. a. Should be factual and accurate. b. Minimal spelling errors and no unacceptable abbreviations. c. Completed in timely manner. d. All patient related encounters (phone, in person) should be documented using athenaNet. e. Billing codes must be verified through appropriate documentation.

7. Maintains credentials essential for practice, to include licensure, certification, and CPE.

8. Attends CVCH Behavioral Health and Medical Provider meetings and represents the organization at any appropriate community committee as directed by the Behavioral Health Director.

Job Specifications

1. Education: Graduate from an accredited School of Nursing. Successful completion of an accredited Advanced Registered Nurse Practitioner (ARNP) program with emphasis in psychiatric care (Nurse Practitioner Certificate or Master's Degree).

2. Certification/Licensure: Current Washington State Advanced Registered Nurse Practitioner (ARNP) license with prescriptive privileges or eligibility to obtain licensure. Valid DEA Controlled Substance Registration Certification. ANCC certification as a "Clinical Specialist in Adult Psychiatric and Mental Health Nursing" or "Adult Psychiatric and Mental Health NP" or "Family Mental Health NP" preferred. Basic Life Support (BLS) required. ACLS or ATLS may be used to substitute for BLS. All Providers must be eligible for enrollment in Medicare and must not be currently opted out of the Medicare program

3. Experience: Two (2) years of experience as a practicing Nurse Practitioner in a clinical setting. Two (2) years experience in providing Psychiatric/Behavioral Health Care to mentally ill individuals preferred. Previous training and experience in community primary health care preferred. Experience working with low income, multi-ethnic populations.

4. Language Skills: English required.

5. Essential Technical/Motor Skills: Ability to communicate both verbally and in writing. Ability to write clear, concise and accurately. Knowledge of computer applications and equipment related to work. Must have basic computer and keyboarding skills and have the ability to enter data within company's computer/software system; must demonstrate manual dexterity.

6. Interpersonal Skills: Strong interpersonal and communication skills and the ability to work effectively with other staff and management and with a wide range of consumers and constituencies and members of the public in a diverse community. Demonstrated skill in developing and maintaining productive work teams. Ability to work in a fast paced environment under stressful situations. Ability to work with and/or around persons who are chronically mentally ill. Skilled in interpersonal relationships. Ability to work effectively in a community health setting utilizing physicians, nurse practitioners, physician assistants, and support staff. An awareness and appreciation for both the health and socioeconomic needs of a low-income, underserved population.

7. Essential Physical Requirements: This job is performed mostly in a typical inside, office environment. Essential physical requirements of this job include: light physical effort; repetitive motions of wrists, hands, and/or fingers; standing, walking, lifting, reaching, kneeling, bending, stooping, pushing, and pulling; frequent sitting; lifting and/or moving items weighing up to 20 pounds; ability to read forms and computer screens and to read correspondence and other documents.

8.
