Nurse Practitioner - Pulmonary - SNF / LTC Rehab center

We are seeking an experienced APRN in pulmonary care to provide high-quality, patient-centered services in skilled nursing and long-term rehab settings. This in-person, part-time role requires availability for 2 days per week, including weekends, with occasional on-call. Provide pulmonary assessments, manage chronic and acute respiratory conditions, and oversee therapies like CPAP/BiPAP and trach care. Admit, evaluate, and discharge patients while coordinating with interdisciplinary teams. Prescribe medications, interpret diagnostics, and monitor treatment responses. Educate patients and staff, participate in care planning, and promote initiatives to reduce hospital readmissions. Qualifications include an active APRN license, Master's degree, experience in long-term care, BLS/ACLS, and strong clinical skills. Knowledge of regulations and EMR systems is preferred.
