Key Responsibilities\n

• Utilize electronic medical records (EMR) to monitor, document, and communicate patient progress toward therapeutic goals, ensuring efficient workflow and documentation.

• Collaborate with a multidisciplinary team, actively participating in interdisciplinary meetings to anticipate patient needs and streamline care plans.

• Educate patients and families on available community resources, support options, and care transitions, ensuring they understand their rights and available services.

• Advocate for patients and families, ensuring access to protective services and timely interventions in case of vulnerabilities or suspected abuse.

• Employ technology to identify and intervene with patients at risk of readmission, aiming to prevent unnecessary hospital stays.

• Plan and participate in transition conferences involving patients and their families, facilitating smooth transitions in care.

• Demonstrate ethical decision-making in all interactions while upholding patient rights and confidentiality.

• Complete additional duties as assigned, contributing to the overall efficiency and effectiveness of the clinical team.

Qualifications\n

Compensation and Benefits\n
