Key Responsibilities\n- \n
- Provide individualized therapy sessions to clients, utilizing various therapeutic modalities to support psychological well-being. \n
- Utilize electronic medical records (EMR) to monitor, document, and communicate patient progress toward therapeutic goals, ensuring efficient workflow and documentation. \n
- Collaborate with a multidisciplinary team, actively participating in interdisciplinary meetings to anticipate patient needs and streamline care plans. \n
- Educate patients and families on available community resources, support options, and care transitions, ensuring they understand their rights and available services. \n
- Advocate for patients and families, ensuring access to protective services and timely interventions in case of vulnerabilities or suspected abuse. \n
- Employ technology to identify and intervene with patients at risk of readmission, aiming to prevent unnecessary hospital stays. \n
- Plan and participate in transition conferences involving patients and their families, facilitating smooth transitions in care. \n
- Demonstrate ethical decision-making in all interactions while upholding patient rights and confidentiality. \n
- Complete additional duties as assigned, contributing to the overall efficiency and effectiveness of the clinical team. \n
This role involves working with a diverse range of clients including individual adults, children, and families facing mental health issues. Therapists will provide services to clients dealing with stress, anxiety, depression, trauma, and more. Qualifications\n- \n
- A Master’s or Doctorate degree in Clinical Psychology, Counseling, or Clinical Social Work from an accredited institution. \n
- An independent license to practice in the state of residence (LCSW, LMFT, LPC) is required. \n

- Ability to manage a diverse caseload while effectively collaborating with multidisciplinary teams and community resources. \n
