Job Details Level: Experienced Location: Simpson Meadows - Downingtown, PA 19335 Position Type: Full Time Education Level: Technical / Trade School Travel Percentage: None Job Shift: Overnight 11pm-7am Job Category: Nurse Duties and Responsibilities The LPN will be responsible for providing direct nursing care to residents in an assisted living or personal care setting, under the supervision of the AL/PC Administrator. This includes administering medications, monitoring vital signs, providing wound care, ensuring resident safety and comfort, and maintaining accurate records. Administer medications and treatments as prescribed by the physician in accordance with Simpson’s medication administration policy. Monitor and record vital signs, such as blood pressure, pulse, and respiration rate. Assist with activities of daily living, including bathing, dressing, and grooming. Provide wound care, including cleaning and dressing wounds. Provide in-dwelling catheter care occasionally when needed. Assist with meal preparation and feeding, as needed. Assist with mobility, transfers, and positioning of residents. Document all care provided and maintain accurate resident records. Communicate effectively with residents, families, and other healthcare professionals. Adhere to all policies and procedures related to resident care and safety. Assist in creating, updating and maintain resident care plans as necessary. Participate in continuing education and training to maintain licensure and stay current with nursing practices through Relias on‑line training as required under PA Regulations on an annual basis. Qualifications High School Diploma or equivalent (GED). If attended secondary school abroad, diploma must be translated to English. Successful completion of an accredited Licensed Practical Nurse training program. Current Pennsylvania LPN License with no judgments or disciplinary action. CPR Certified. First Aid Certified. AED Certified (Preferred). 1 year experience working as an LPN in long term care. Experience in a memory care facility is preferred but not required. Knowledge of nursing practices and procedures. Effective communication skills with residents/patients, families, and healthcare providers. Team player who follows directions. Ability to handle stressful situations and respond appropriately.
