Heartline Staffing is recruiting a Primary Care Physician to provide preventive primary care for adults 65+, with a focus on Medicare Advantage, chronic disease management, and preventive care. Physicians see approximately 1215 patients per day with 2040 minute visits. Value-Based Care experience is welcomed but is not required; training is provided.

Position Overview

• Adult 65+ patient population
• Majority Medicare Advantage
• Chronic disease and preventive care focus
• Approximately 1215 patients per day
• 2040 minute patient visits
• No RVU pressure
• Integrated care team including NP/PA, Medical Assistants, Social Workers, lab and pharmacy support, and referral coordination

Compensation

• Public posting range: $235,000$260,000 base salary
• Commencement bonus up to $75,000, varying by clinic
• Annual performance bonus: 20% of base salary
• Total first-year compensation: $300K+ depending on experience and performance
• Early sign-on bonus opportunities
• Relocation assistance available for moves greater than 50 miles

Role

• Provide primary care for adults 65+
• Focus on chronic disease management and preventive care
• Practice within an integrated care model
• Work collaboratively with NP/PAs, Medical Assistants, Social Workers, lab and pharmacy support, and referral coordination

Requirements

• Board Certified or Board Eligible in Internal Medicine or Family Medicine
• Comfortable practicing within a value-based care model
• Strong communication and team-oriented mindset

Preferred

• Geriatric experience
• Value-Based Care experience
• HEDIS experience is a plus
