We are seeking a dedicated Registered Nurse (RN) to join a busy ambulatory care team in Lynn, MA. The Primary Care Nurse provides direct patient care, health education, care coordination, and clinical support to patients and families within an outpatient setting.

This role involves collaborating closely with providers and support staff, tracking at-risk patients, administering medications and immunizations, performing point-of-care testing, and helping ensure efficient clinic operations.

In addition to clinical responsibilities, the RN may participate in community outreach initiatives, including health education programs, screenings, and consultation services for local schools, daycare centers, and community organizations.

Qualifications

• Graduate of an accredited nursing program
• Current Massachusetts RN license
• Current BLS certification
• Strong attendance record and positive professional attitude

Preferred Experience

• Community Health Center, Emergency Room, or Ambulatory Care experience
• Experience working with diverse patient populations
• Prior outpatient nursing experience preferred

Key Responsibilities

• Triage patients by phone and in person, determining appropriate level of care and disposition
• Administer and refill medications in accordance with provider orders and organizational policies
• Administer vaccines following CDC, MDPH, and facility guidelines
• Maintain infection prevention and control standards
• Perform patient assessments and clinical procedures
• Provide patient and family education regarding diagnoses, treatments, follow-up care, and home care instructions
• Collaborate with providers and care team members to ensure high-quality patient care and clinic flow
• Participate in community outreach and health promotion activities as assigned
