The RN Homecare role involves providing nursing care to patients in their homes, ensuring they receive high-quality medical support while maintaining their comfort and safety. This position requires strong clinical skills, excellent communication abilities, and a compassionate approach to patient care in a home setting.
Responsibilities
- Assess patients’ health conditions and needs during home visits
- Develop and implement individualized care plans
- Administer medications and treatments as prescribed
- Monitor patient progress and report changes to healthcare providers
- Provide education to patients and family members on disease management and care techniques
- Coordinate with other healthcare professionals to ensure comprehensive care
- Maintain accurate and detailed patient records
- Adhere to safety and infection control protocols in the home environment
- Asses OASIS cases
