What You Will Do

• Practice independently in the community as part of an interdisciplinary care team.
• Conduct comprehensive patient assessments and develop individualized treatment plans.
• Deliver personalized home-based physical therapy services.
• Monitor patient progress and adjust plans of care as appropriate.
• Educate patients and caregivers to support recovery and independence.
• Collaborate with physicians and interdisciplinary team members.
• Manage your own flexible schedule while meeting Guaranteed Flex Pay role expectations.
• Utilize innovative technology to efficiently document patient care.

Qualifications

• Minimum of one (1) year of Physical Therapy experience preferred, or successful completion of the VNS Health Rehab Intern program.
• Current New York State Physical Therapist License and Registration required.
• Valid driver's license may be required based on assigned territory.

Pay Range per Visit:

• Bronx & Westchester: Weekday visits - $105 | Weekend visits - $110
